Hi team,
Margaret Whitlock (84, Rm 14, Banksia Wing) had an unwitnessed fall in her ensuite at 0640 on 12 March. GP review same morning — no fracture; small skin tear to left forearm, cleaned and dressed.
She's been increasingly unsteady on transfers this week and declining to use her 4-wheel walker. Appetite is down — she refused two meals yesterday. New Webster pack started (perindopril added for BP). Occasional nocturnal incontinence noted overnight.
Current FRAT score 14 (medium falls risk). Daughter Helen has asked for a care conference. Please draft an updated care plan for RN sign-off.
