During the 2-hourly check at approx. 02:40 I found M.T. on the bedroom floor beside the bed. He was conscious but distressed and unable to stand, complaining of pain in the right hip. The fall was not witnessed — last check at 00:40 was unremarkable. I did not move him, called the on-call nurse, then dialled 000. Ambulance attended and transported M.T. to Northern Hospital approx. 03:25. Suspected fractured hip. I notified the house coordinator and M.T.'s sister (listed as primary contact). Incident form F2 completed at end of shift.
