Community access & wellbeing check
Met M.R. at the Broadmeadows hub for community access. M.R. travelled independently by bus for the first time — a target from the support plan (Goal 2: build travel confidence). Reported feeling "more settled" this fortnight and that sleep had improved.
Attended the Tuesday social group and connected with two new members. Still anxious about the upcoming NDIS plan review on 26 March.
Mood noticeably lower when discussing finances — missed a Centrelink appointment last week and is worried about a bill. Agreed to a referral to the financial counsellor.
Next session: support to prepare plan-review documents.
